I've had this problem several times and it always turned out to be a bad ground. Run another ground from the battery to the body in addition to the one that is there and see what happens.

Use caution when pulling off a battery cable with the car running. This worked great on older vehicles without all of the electronics that are found on new cars. That is not a great way to test the alternator anymore. I'd double check the wiring going to the alternator and check your battery...
If it is an automatic, try putting it in neutral to start it. If it is a manual, check to make sure the clutch safety switch is working right. Double, no triple check the ground. That is an all important part that most people overlook. Does the car have a security system on it?
Is the smoke white or blue? White smoke is water. Blue is oil. Sometimes its hard to tell... Any water in the oil or oil in the water? Check the oil level too! I know this may seem simple, but overfilling can cause smoking as well.

Well... The issue you have to consider is that the bearings in the pulleys have been spinning just as long as the water pump and the timing belt. I would replace them all. I know you are planning a build, but its always better to be safe than sorry. Just my 2 cents.
That could be lifter tick or coming from the rail or injectors. I'd start with a fuel pressure test and then do a compression test if the fuel pressure passes. You can rent a tester from Autozone. Let me know what you find out.
