If you got these direct from Delta they should have come with shims or as bogus states #10 washers. If you didn't shim them than thats most likely the source of your noise.

Correct it needs to be flipped 180. It should sit inside a grove on the TOB at least on the OEM ones that does not look like an OEM TOB though. I also highly doubt that the clip will cause your clutch not to disengage. While you had it on backwards it was still doing its job which is holding...
