Once you get the tb off and apart, you'll see two small ports on the lower section towards the end. One is for the IAC, the other is for the FIAV. You want to block the one that isn't for the IAC. If you're still not sure, search around for FIAV, there is a link to a picture in one of them that...

I've got similar issues with my idle. At cold startup, it'll rev to 2k for a few seconds, then drop down into a surging idle untill it warms up. If I start it when its already warm, it idles fine. I just pulled the tb off and apart tonight and used JB weld to block off the hole in the bottom...
