Make sure you don't have any boost leaks anywhere from the turbo compressor housing to the intake manifold gasket, make sure you dont have any exhaust leaks anywhere before the turbo including the wastegate. You shouldnt have that problem on a 16g unless there is something critically wrong with it.

You can use a diamond hole saw to cut a hole in the intake pipe then braze an aluminum pipe in if you cant weld aluminum. Make sure the pipe and hole saw are the same size as you bov outlet or very similar.

The car will run a bit rich with the 660cc injectors, you could get away with 550cc injectors and a 2g maf without any tuning software most likely. I dont think the 14b will flow enough to cause your bov to leak, if it does then crush it untill it almost touches the nipple.
If your spark plug gap is .9mm which is .035in that could be causing your engine to miss and pick it up as knock. It should be more around .024in to .032in which is .6mm to .8mm. Don't know what your log looks like since i'm on mobile. Just a possibility.
When you say spikes at 13lbs does it hold the boost steady or drop lower than that if you keep in boost? If so make sure your airfilter is not collapsing.
