90 gsx pops out of first gear when clutch is released??

hi, my 90 gsx started to do this about 3 weeks ago. if you put the car in first gear and let off the clutch the car would slip out of gear and make a horrible grinding noise. it is bad to the point now that you have to hold the shifter forward with all of your strength just to get the car to stay in gear when taking off. i've started taking off in 2nd gear but 2nd will also occasionally do it too. also, another problem that may or may not be related is that the clutch (or something) is slipping at above 4000 rpm. even on light acceleration the clutch will slip in gears past 3rd and 4000 rpms. i figured i would need a clutch either way so today i ripped the tranny out to inspect everything. the clutch doesn't really look that bad, there is a decent amount of material left on the plate (not sure if this means anything or not) i already bought the clutch so i'm going to install it either way but is there anything else the problem could be? should i be looking at anything else since i have the transmission ripped out? any help is GREATLY appreciated!
 
Mine did this is 2nd gear. I took the end cap off and the bearings on the bottom shaft were shot and the whole shaft would jump, causing the grinding. Needless to say, my tranny is out and I'm in the process of putting a 91 GST tranny in it.

Pull the end cap off and see if you have any up and down play, also if your fluid has a lot of metal in it thats a sure sign too of tranny death...
